Please complete this questionnaire in order to qualify for certification. The results of this questionnaire will be emailed to you. The results are confidential and not shared or viewed, so please be honest in your responses.

We suggest you meditate on who you were at the beginning of your Magnum Opus journey. Perhaps look back at the answers from your questionnaires completed before you started each stage. What were the obstacles and frustrations you experienced, how were your relationships to others and yourself, how did you feel about the world?
